WAVE’s 15th Anniversary Sweepstakes

Official Rules and Regulations

  1. No contribution is necessary to enter or to win a prize in this sweepstakes. A donation will not improve your chances of winning.

  2. Only one entry per person is allowed. Void where prohibited.

  3. Open only to legal residents of Wisconsin. Must be 18 years or older (as of October 22, 2012) to participate.

  4. Odds of winning depend on the number of eligible entries received.

  5. The Sweepstakes begins at 12:01 a.m. CDT on October 22, 2012 and ends at 11:59:59 p.m. CDT on November 8, 2012 (the "Sweepstakes  Period"). All online entries must be received during the sweepstakes period to be eligible to win a prize. All mailed entries must be postmarked by November 8, 2012 and received by the sponsor by November 12, 2012 to be eligible to win a prize.

  7. To enter WAVE’s 15th Anniversary Sweepstakes, you must do only ONE of the following:
    1. Donate to WAVE’s 15th Anniversary Event on any webpage specifically identified by Sponsor as a WAVE’s 15th Anniversary Sweepstakes webpage during the sweepstakes period.
    2. Return an official WAVE’s 15th Anniversary Sweepstakes sponsor form during the sweepstakes period.  Form must be postmarked by November 8, 2012 and received by November 12, 2012. Forms should be mailed to: WAVE Ed Fund, P.O. Box 170393, Milwaukee, WI, 53217.
    3. Send a postcard with your name, address, phone, email address (if you have one) and the words “WAVE’s 15th Anniversary Sweepstakes” written on the postcard to: WAVE Ed Fund, P.O. Box 170393, Milwaukee, WI, 53217. Postcards must be postmarked by November 8, 2012 and received by November 12, 2012.   
  1. Prize. One (1) winner will receive the following prizes ("Lunch with Barrett "):
    1. Lunch for winner and a guest with Mayor Tom Barrett, on a date and at a venue to be determined by the Sponsor in its sole discretion (approximate retail value $60.00).  Notice: Prize does not include alcoholic beverages, travel expenses or incidental expenses.
    2. A Copy of the book  Milwaukee 150: The greater Milwaukee story (approximate retail value $25.00). 
  1. Winners will be selected on November 12th 2012 .
    1. Twenty Five (25) potential winners will be selected by a random drawing from all eligible entries to be held at WAVE Educational Fund on November 12, 2012.  Sponsor may, at its option, conduct a background check on each potential winner. Sponsor reserves the right to disqualify any potential winner from receiving any prize if Sponsor determines, in its sole discretion that awarding any prize to such potential winner could result in a safety or security risk to any person or persons or could result in the disruption of any event associated with the Sweepstakes. Sponsor in its sole discretion, will randomly draw one (1) finalist potential winner from the list of eligible potential winners.

    2. The winner will be notified by phone or e-mail by November 13, 2012. To claim the prize, each winner should follow the instructions and comply with the conditions contained in his or her notification. Winners must then identify potential guests to Sponsor, and Sponsor may, at its option, conduct the same background check and process applicable to potential winners. Sponsor reserves the right to disqualify any potential guest in its sole discretion. If a finalist potential winner cannot be reached after 5 days from first notification attempt, or if an entrant is found to be ineligible or are otherwise disqualified, such prize will be forfeited and an alternate winner selected. 
  1. The winner will be posted at waveedfund.org on or about November 23, 2012 and will remain posted no less than 14 days. If you would like a list of the 2012 Fall Sweepstakes winner sent to you, mail a self-addressed stamped envelope to: WAVE ED FUND Winner List, PO Box 170393, Milwaukee WI 53217.

  2. Employees of WAVE Educational Fund, their service providers, those involved in the development, production, implementation and distribution of this sweepstakes, including their officers, directors, employees, advisors, assignees, agents, licensees, representatives, and any other person/entity associated with WAVE Educational Fund’s Anniversary Sweepstakes including their immediate families and members of their same households, are ineligible for sweepstakes prizes.

  3. Once submitted, entries become the sole property of WAVE Educational Fund and will not be returned.

  4. Winners are responsible for all costs of prize redemption and/or travel to use a prize, or any taxes which may be applicable. The value of each prize is determined by the prize donor or vendor and such determination is final for all purposes. Winners may waive their right to receive prizes. Prizes are nontransferable.

  5. By entering, entrant agrees to be bound by these Official Rules and all applicable local, state and federal laws. Winners will be required to sign a publicity release and a winner’s affidavit to claim any prize.

  6. WAVE Educational Fund and its entities and staff are not responsible or liable for multiple entries, entries completed or entered by other than human means, or entries that are late (including delayed data transmission), tampered with, illegible, mutilated, forged, destroyed, incomplete, garbled, lost, misdirected, mechanically duplicated, mass machine printed, postage-due, or otherwise not in compliance with Official Rules – and all such entries will be disqualified.
