Please complete the fields below and attach the form that you receive in the email to the artist's work when you mail it in. (Your information is necessary so that we may contact semi-finalists & winners.) The email form that you receive will have blank spaces for you to fill in information about the artist and the work of art. 

When you receive the form by email (check your spam if you don't receive it within an hour) please:

1) Print as many copies of the form as necessary (one for each artist).

2) Fill out the remaining information that corresponds to each artist and his/her artwork. 

3) Please be sure to attach the form to each corresponding work of art. 

4) Please be sure to write Do Not Bend on the envelope!
